You and your provider will discuss and create a plan for improving your health and preventing disease. Wellness visits are suggested once yearly, although your specific health needs may require more frequent visits.
Medicare patients are eligible for an annual wellness visit at no additional cost.

Acute Care
Acute care situations occur when you receive active but short-term treatment for a severe injury or episode of illness, an urgent medical condition (e.g., flu, allergies), or during recovery from surgery. Integrated Health Centers of West Virginia will help you through an acute care incident as it occurs.
You can schedule an appointment by calling or texting 304-643-4005 or visiting any of our eight campuses. We proudly offer same-day medical appointments and accept walk-ins. Please note that scheduled patients are seen first, but we do our best to accommodate walk-ins as soon as possible. Due to scheduling needs, we cannot accept walk-ins for our dental and behavioral health services.

Chronic Care Management (CCM)
CCM is a critical component of primary care that contributes to better outcomes and higher patient satisfaction. CCM services include interactions with patients by telephone or secure email to review medical records and test results or provide self-management education and support that precludes medication management. Each patient will be provided with a comprehensive care plan.
CCM services also include interactions with the patient’s other healthcare providers (HCP) to exchange health information, manage care transitions, and coordinate home and community-based services. Some examples of chronic conditions include but are not limited to arthritis (osteoarthritis and rheumatoid), asthma, cardiovascular disease, chronic obstructive pulmonary disease (COPD), depression, diabetes, and hypertension.
IHCWV recognizes the population we serve may consider an additional service, such as the CCM program, a financial burden. Therefore, IHCWV has accounted for the CCM program in the IHCWV Sliding Fee Discount Program. Patients will follow the same process already in place to apply for the IHCWV Sliding Fee Discount Plan. Once documentation is received and the application approved, IHCWV Sliding Fee Discount Plan eligibility will be effective for 12 months.
If you want more information or would like to speak with the Chronic Care Coordinator to see if CCM can benefit you or a family member, please get in touch with us by calling or texting 304-643-4005.

Preventative Screenings
Early detection is the best protection against serious health problems. Regular check-ups and screenings can help catch potential issues before they become significant problems. Don't wait until you feel sick to see one of our healthcare providers. Take charge of your health and schedule your next screening today!
Breast and Cervical Cancer Screenings:
Prostate Exams:
The American Cancer Society recommends that men have a chance to make an informed decision with their healthcare provider about whether to be screened for prostate cancer. After getting information about the possible benefits, risks, and uncertainties of prostate cancer screenings, the decision should be made.
The discussion about screening should take place at:
After this discussion, men who want to be screened should get the prostate-specific antigen (PSA) blood test. The digital rectal exam (DRE) may also be done as a part of screening. If, after this discussion, a man cannot decide if testing is right for him, the screening decision can be made by your healthcare provider, who will consider your general health, preferences, and values.
Diabetic Eye Exams:
The American Diabetes Association recommends that adults with type 2 diabetes have their first eye exam when diagnosed, and people with type 1 diabetes have their first eye exam within five years of diagnosis.
Breast and Cervical Cancer Screenings:
- Mammograms: Women ages 40 to 44 should talk to their healthcare provider about when to start annual mammograms. Women 45 to 54 should get mammograms every year. Women 55 and older can switch to mammograms every other year, or they can choose to continue yearly mammograms. Screening should continue as long as a woman is in good health and has a life expectancy of 10 years or more.
- Pap tests: Women ages 21 to 29 should get a Pap test every three years. Women ages 30 to 65 can choose to get a Pap test every three years, or they can get an HPV test every five years, or they can get a Pap test plus HPV test every five years. Women 65 and older who have had normal results for the past decade can stop cervical cancer screening.
- Additional Considerations
- Family History: If you have a family history of breast or ovarian cancer, you may need to start screenings earlier or get them more frequently. Talk to your doctor about your family history and what screenings are right for you.
- Risk Factors: If you have other risk factors for breast or cervical cancer, such as smoking or a history of sexually transmitted infections, you may also need to get screened more frequently.
- Age 50 for men who are at average risk of prostate cancer and are expected to live at least ten more years
- Age 45 for men at high risk of developing prostate cancer. This includes African American men and men who have a first-degree relative (father or brother) diagnosed with prostate cancer at an early age (younger than 65)
- Age 40 for men at even higher risk (those with more than one first-degree relative who had prostate cancer at an early age)
After this discussion, men who want to be screened should get the prostate-specific antigen (PSA) blood test. The digital rectal exam (DRE) may also be done as a part of screening. DOT Physicals
A Department of Transportation (DOT) physical is a physical exam that is required of people who drive commercial vehicles. The point of a DOT physical is to make sure that people who are driving commercial motor vehicles (CMVs), such as large trucks and buses, are physically, mentally, and emotionally fit. The Federal Motor Carrier Safety Administration (FMCSA) requires this testing for the safety of the public.
If your employer requests a DOT physical, IHCWV has multiple providers who can perform the exam. Contact us to schedule an appointment.
