Aledade is our Accountable Care Organization (ACO). Their job is to ensure we meet quality standards for our patients, especially our Medicare population. They help us with medication adherence and call our patients when they are late picking up medications to assess any barriers that the patient may be experiencing. He will provide primary care services to patients of all ages in Smithville and St. Marys.
Originally from Glandorf, Ohio, Tyler recently graduated from the University of Findlay, where he studied Biology. It was during his time there that he met his wife, Heather, who graduated from Williamstown, WV. They have two sons and are excited about settling down in West Virginia.
